Why is my 5 month old puppy still peeing in the house?

I have an Aussie/Husky Mix. She has had the #2 thing figured out at week 10. I could count the number of times she has done #2 in the house on my hands. Which is great. But the Pee thing is frustrating. She goes about once a day. Today, however, she has gone over 5 times as if I have never trained her anything! It is SO frustrating. I have read other blogs and do/try it all and it seems like nothing works! HELP!
I just want to add…we never yell as a “found” accident because I read you shouldn’t do that. I take her out when I see it. I always praise and I am always watching her. She manages to slip a pee literally when I turn around. I watch her so carefully. I have done everything, but hit her with newspaper. The only advice I have not done.

The keys to potty training in any dog are

Supervision – CONSTANT and vigilant supervision at all times. If this means leashing the dog to you and putting her outside the second she starts to sniff around, then so be it!
Consistancy – every 30 minutes plus immediately after eating, drinking, sleeping, or playing
Be realistic – realize that the dog can’t hold it for hours, needs help to learn, and won’t pick it up overnight
Reward – praise not punishment
Be Patient
